Hello,
I've been wanting to make my own blank dermal tops to put my own cabochons in, but I'm not too sure where to begin.
I've looked into machining, casting, and I've even looked into jewelry and metal smithing forums on Reddit and can't seem to get a clear start. Any information about about crafting dermal tops would be helpful!

I'd like to help, but I don't understand what additional direction you need. I suppose your easiest first step would be taking a local silversmithing class and learning to mount cabachons from that. Steel is significantly harder to work with. You said you already looked into smithing though, so I don't want to give you advice that you've already considered... if you can clarify what you're looking for, I'm glad to assist further.

I cant promise he would tell you all of his secrets, but Alan of Origins and Bella Piercing makes a lot of intricate, tiny pieces. I dont know everything hes done to learn metal smiting and gem work but he does have a background making other types of jewelry (natural materials/PTFE) and I believe he took a few classes at a local university to get started in metal work.
